Registration Forms

After a student has applied to the Academy, we require some student supporting information by way of completion of the four mandatory forms detailed below.  The quickest way to do this is by logging on to our Digital Data Collection website ddc.lfatsf.org.uk   (note: should a student not join the Academy, this information will be deleted immediately):-

If the student requires a place on the academy minibus, please complete the Academy Transport Application Form

To apply for student support, please complete the Student Support Scheme (Discretionary Bursary and Free Meals in Education) Form

Should your details change at any time, please complete the Change of Student Details Form
